| VITAL RECORDS
BIRTH CERTIFICATES:
• Birth certificates are available for any person born in
Chicago Heights or at St. James Hospital and Health Centers located
in Chicago Heights. Birth records date back to 1869.
• Birth records are released only to the parents listed on
the birth record of a child born at St. James Hospital or to the
person if he or she is at least 18 years old. A person requesting
a birth certificate must fill out a request form and show a driver’s
license or state identification. If the person does not have these
forms of ID, alternative ID must be presented.
• The fee for a birth certificate is $10.00 cash or money
order for the first copy, and $5.00 for each additional copy of
the same birth certificate.
• Personal checks are not accepted for vital records.
Birth Certificate Application
DEATH RECORDS:
• Death records are available for any person who died in Chicago
Heights, regardless if the deceased resided in another city. Death
records date back to 1902.
• Immediate family members of the deceased may request a death
record upon completing a request form and showing proper identification.
Anyone having a legal interest in the estate of the deceased may
request a death record with proof of interest and proper identification.
• The fee for a death record is $10.00 cash or money order
for the first copy, and $5.00 for each additional copy of the same
death record.
• Personal checks are not accepted for vital records.
Death Certificate Application
ADDITIONAL VITAL FORMS:
• Birth Correction forms are available at
the Clerk’s Office. If an error is found on the birth certificate,
this form may be filled out and mailed to Springfield Vital Records.
There is a $15.00 fee per four corrections and the time limit depends
on the corrections being made.
• Voluntary Acknowledgement of Paternity
forms are available at the Clerk’s Office. If the father’s
name is not on the birth certificate, this form must be completed
by both parents and submitted to Springfield Vital Records so that
the father’s name may be added to the birth certificate.
• Voluntary Denial of Paternity forms are
available at the Clerk’s Office. If incorrect paternal information
appears on the birth certificate, this form must be completed by
both parties and submitted to Springfield Vital Records so that
the incorrect information can be corrected or removed from the birth
certificate.
• The Birth and Death Registry is available
at the Clerk’s Office. The addresses and phone numbers to
all Vital Records Departments for the United States, Trust Territory
of the Pacific Islands, and the Virgin Islands are available on
this list.
• Springfield Vital Records has a special
service for those individuals who need vital records immediately.
There is a $34.00 charge and a Visa or MasterCard is needed. The

• Chicago Heights Vital Statistics are available
at the Chicago Heights Municipal Building, 1601 Chicago Road, Chicago

• Cook County Vital Statistics are available
at Markham Courthouse, 16501 Kedzie Avenue, Markham, IL 60426; 708-

• Illinois Vital Statistics are available
